New Patient Registration
As of 1st June 2022 we are unable to accept new patient registrations from patients who already have a local GP.
We will only be accepting new patient registrations from patients who are new to the area and do not already have a local GP.
We are happy to welcome any new patients to register with the practice. Please see if you are in the practice's catchment area.
If you live within our practice area, to register with the practice please ask for a registration form from Reception or complete the electronic form. (Please note if you complete the online form you will need to come in to sign the form and show us some form of ID).
You can either complete the pre-registration form (GMS1) online below or access the form online.
All newly registered patients are invited to attend the Surgery within a month of joining for a general health check with our Nurse. This allows your medical history to be summarised whilst awaiting the arrival of your notes from your previous Doctor.
The New Patient Check comprises a blood pressure, height and weight measurement and a urine screen for diabetes.
