Repeat prescriptions

If you require a prescription for medication you take regularly, you may either:

Email it to the Surgery.
Leave your request in the box at the Surgery entrance.
Telephone (015395) 36366 and press '1' for the dispensary or hold for the receptionist. Alternatively, leave a message on the 24-hour answer machine if calling out-of-hours.
Fax it to (015395) 36766.

Please allow two full working days.

How to register to use the Internet Appointment Booking and Repeat Prescription Facility

To use the Internet Appointment Booking and Repeat Prescription facility you need to register on the web site at www.patient.co.uk.

There are two registration screens. The information you enter on the first screen enables the system to check your identity when you log on. On the second screen you enter a password and security details that no-one else will know.

The information you need to complete the registration screens is available as a PDF download here. Look after this form carefully until you register, and consider destroying it afterwards. If you lose the form or if it is stolen before you have entered your password and security details, contact the practice straight away, by telephone or in person.

Remember to enter the information exactly as it appears on this form, or your registration will not be accepted. When you log on after registering you must also enter all details in exactly the same format.

Once you have accessed the screen you will be able to book an appointment with the Doctor, inform us of Changes of Address/Contact Number and order your repeat prescription. Please only book routine appointments. If you need an appointment for a smear, minor operation or any medical assessment, such as a driving medical, please contact the surgery in the usual way.

Please note

Let your GP or Pharmacist know if you've stopped taking any of your medicines.
Check what medicines you still have at home before re-ordering.
Discuss your medication with your Pharmacist or GP on a regular basis.
Think carefully before ticking all the boxes on your repeat prescription forms and only tick those you really need.
If you don't need the medicine now - don't order it! If you need the medicine in the future you can request it then.
If you need to go into hospital, please take all your prescription medicines with you.
